Trauma was caused by a bike accident on this 12-year-old patient. Two upper centrals, both complicated crown fractures, both with incomplete root development. #8 came in with the piece; #9 did not. I did a Cvek partial pulpotomy on both with Biodentine.
Preop, then the pulpotomies. For #8 I used green OpalDam to provide a custom matrix before reattaching the fragment.

Postop films the same day, and the teeth restored.

Recalls at 18 months and 2.5 years. Both teeth are in braces now, and there is no staining.

Takeaway
With or without the piece, an immature tooth with an exposure still gets my go-to, the Cvek partial pulpotomy. Both of these got it with Biodentine, and at 2.5 years there is no staining. The dentin bridge between the Biodentine and the pulp is evident at 18 months (blue arrows). Both teeth tested positively to cold. Some alveolar remodeling could be seen at 18 months which has not progressed in the following year.
